Vital Statistics

Welcome to the Portsmouth City Health Department Vital Statistics. Any questions, comments, or information you need, please call (740) 353-5153 Ext. 224

 

The price of each certified copy or abstract of a birth or death certificate is $25.00 per copy.

There are two ways in which to obtain a birth or death record in Ohio. Please choose the method that best suits your needs:

  1. Walk-in Same day service is available to walk-in customers. This is the fastest way to obtain a birth certificate. When you arrive, you will complete an application and pay the $25.00 required fee. Payment can be made by cash or check with a picture ID only (no debit/credit cards accepted).   2. Mail Turn around is estimated at 1 to 2 weeks from the day the request is received. However, people are urged to allow sufficient time for delivery for all birth/death records. Mail a money order of $25.00 for each certified certificate. Cash or personal checks are not accepted through mail request.  Self-addressed stamped envelope (a prepaid overnight service can be used if the record is needed quickly) is required.Mail your request to:

Your area code and daytime phone number

 

A money order of $25.00 (in U.S. funds) for each certificate (birth and death only), payable to “Portsmouth City Health”, should accompany each request which covers the cost for the certificate and search.
